Am I Eligible for Hospice Care?

You may be eligible for hospice care if:

• You have a serious or terminal illness with a prognosis of six months or less if the disease follows its expected course
• Your physician and hospice medical director certify eligibility
• You choose comfort-focused care rather than aggressive or curative treatments

Common conditions that may qualify for hospice include:

• Cancer
• Heart disease
• Lung disease (COPD)
• Kidney failure
• Dementia or Alzheimer’s disease
• Stroke or neurological conditions

Eligibility is reassessed regularly, and hospice care can continue as long as the patient meets the criteria.
